Place bread in a large bowl with milk. Let soak while you are working on the rest of the meatloaf.
Cook onions, carrot and garlic in a skillet until vegetables are soft. Set aside to cool.
Combine beef, eggs, herbs, salt, pepper, and cooked vegetables to the soaked bread mixture. Mix until combined well.
Press into a loaf pan and bake in a 400 degree oven for 45 minutes. If you don't have a very lean beef, you will need to pour off grease before adding the glaze. If you use a grass-fed beef, you won't have any fat to pour off.
Mix together glaze ingredients in a small bowl and pour over the top.
Bake for an additional 15-20 minutes.
Let rest for a few minutes, slice, and enjoy!
